Mission: Youth Outreach Parent/Guardian Submission Form

Welcome & thank you for your service to our great nation!

We’re excited to introduce our Boys & Girls Clubs to your military family through the Mission: Youth Outreach (MYO) initiative, which provides a stipend to a local Boys & Girls Club to provide a free or reduced annual membership for your eligible child once annually.*

Your youth are eligible for this Mission: Youth Outreach initiative stipend if they are a dependent of a military sponsor who is an active duty, national guard or reserve component. Our local Club programs are designed to inspire and engage your children, kids and teens. By filling out this form, you’re taking the first step towards providing your kids with a safe, fun, and educational environment where they can thrive. We look forward to partnering with you to support your child’s growth and development. Please complete the interest form below and you will receive an email with further information and instructions.

*Additional fees for membership, transportation or other programs may apply at some local Boys & Girls Clubs. Supplementary scholarships may also be available at the Club upon local registration.

    Clubs are listed under their parent organization name. Click the website in your search results to find the correct name to select from the dropdown.
  2. Toggle back to this enrollment form, then select State from the drop-down menu below
  3. Select Club from the drop-down menu in enrollment form below
    • If Club is listed, complete enrollment form below
    • If Club is NOT listed, this means they have not yet opted in to the Mission: Youth Outreach initiative. You may print out Join MYO flyer and take to your Club of choice or send an email to MYO team with Club name and we will ask the Club to join the Mission: Youth Outreach program!
